WHAT IS 'THE SMILE PLAN?'

THIS PLAN IS NOT INSURANCE AND IS NOT PRESENTED AS SUCH.

Instead of dealing with complicated insurance with loopholes and coverage that sends you in endless circles, try our very simple plans. Once you pick the plan that works for you, all you have to do is sign up, call and make your appointment, and take advantage of the massively discounted prices for over 100+ procedures for all ages, wrapped into one yearly cost, saving you and your family and friends potentially hundreds of thousands while taking care of your smiles! 


This plan is not a Qualified Health Plan under the Affordable Care Act. This plan provides discounts from the dental provider to the patient for dental services.

SPECIALISTS

There are procedures that are not provided by the dental office, and the patient will need to go to a specialist. We refer out to a specialist when we feel that additional treatment is needed, or the procedure requires the additional skill of a specialist to complete.


Any services or costs that require a specialist will not be provided by this plan. 

This is not an insurance plan. It is a cost reduction plan, so the patient does not have to pay full costs for basic dental services from the provider. These services are listed in the Schedule of Treatment. The patient may request to see the Schedule of Treatment.

The patient is responsible for any costs at the specialist's office. 

We have specialists that we typically refer to and will notify the patient of our preference. Each patient's case is unique, and it is up to the specialist's office if they will reduce their costs.

RENEWALS AND UPDATES

This plan is for a 12 month period. 

Renewals can be made on or before the expiration date for services to continue uninterrupted. Coverage expires immediately after the expiration date. Coverage resumes immediately after the plan is renewed. It is the patient's responsibility to renew.


Dental work started and (or) paid for before the Plan coverage expires will be completed with the Plan discount, even if the Smile Plan has expired before work is completed. This includes an appointment made PRIOR to the expiration, even if the actual appointment falls AFTER the plan expires due to scheduling. However, every effort will be made to schedule the appointment while the plan is active. 


Only work completed and paid for at THIS appointment will be completed with the Plan discount. Any additional appointments will require renewing the plan to receive the discount.


Any Treatment Plan quotes based on enrollment in the Smile Plan will be continued at full price unless the plan is renewed prior to treatment. 

Treatment Plan quotes received while on the Smile Plan are based on active enrollment in the Smile Plan and current enrollment is required to receive the discount.


Members need to notify the office of ANY changes in their contact information. Updated information is critical for reminders of appointments and Smile Plan expiration dates. It is not the Smile Plan's responsibility to provide these reminders, but it is provided as a courtesy.

WARRANTIES

All dental work is guaranteed up to 3 years to be replaced free of charge, with the exception of lab fees and charges, as long as the patient makes their checkups every 3 or 6 months or as scheduled depending on treatment.


Missed appointments voids this warranty on all dental work. No exceptions.

This warranty does not cover: damages due to accidents, or neglect, or acts of nature including the patient's physical attributes or health, or any actions of the patient not in compliance with the doctor's instructions.


This warranty only covers work provided by contracted doctors using the Smile Plan.

CHANGES IN THE PLAN

Smile Plan reserves the right to change the price for the dental services to accommodate the rise in costs in the dental industry.
All participants will be notified of any changes in the plan.
Price changes will be available on the Schedule of Treatment. You may request to see the Schedule of Treatment.
Please keep all contact information updated to aid in your right to access changes in your plan.
Smile Plan bears no responsibility if your right to access changes is impeded by incorrect contact information.

REFUNDS

You have the right to cancel your Smile Plan membership within 30 days after submitting payment to receive a refund, less a processing fee of $50, by submitting a request in writing to Smile Plan at 670 Colonial Rd Suite 6, Memphis, TN 38117.


No refund will be given if the patient has used the plan IN ANY CAPACITY, including to receive an exam, x-rays, or dental work.

We offer over 100+ procedures including: orthodontics (braces and corrective work), cosmetic services (whitening, veneers, porcelain crowns), fillings, root canal treatment, tooth removal, pediatric work (children's dental treatment), and even certain emergency cases amongst the COVID-19 pandemic. WHAT ARE SAME-DAY VENEERS? DO YOU OFFER THEM?

Same-day veneers are made up of composite resin material and are generally less expensive than porcelain veneers but require more follow-up visits. Yes, we do offer them! Give us a call and set up your appointment today!
